Forensic Diagnosis of Non-Consensual Fellatio in A Child in A Public Transport Setting: A Case Report

Background: Sexual violence in public transport is a widespread global concern disproportionately affecting women and girls. Most reported incidents involve non-contact or less severe forms of sexual harassment, whereas serious sexual assaults in public transport settings are considered uncommon. This report describes a rare case of forced oral sexual assault involving a minor in a public transport setting.
Case presentation: A 14-year-old girl was referred for sexual assault forensic examination following a complaint of suspicious behaviour reported by a bus conductor. During the initial interview, the child denied oral sexual contact. Oral examination revealed multiple petechiae over the soft palate, while microscopic examination of smears prepared from oral swab specimens confirmed the presence of spermatozoa. Following reinterview, the child disclosed coerced oral-genital contact during the bus journey. Based on the history, clinical findings, and laboratory results, the findings were considered consistent with a forced oral sexual act. A medico-legal report was submitted to support legal proceedings.
Conclusion: This case highlights the diagnostic challenges of non-consensual oral sexual assault and emphasizes the importance of prompt forensic examination and timely specimen collection.
